A former supervisor at the Northeast Human Service Center in Grand Forks accused of forging prescriptions to get painkillers has signed a plea agreement.
Tami Byzewski, 39, pleaded guilty Thursday in state District Court to two counts of illegally obtaining a controlled substance. She's set to be sentenced Feb. 17.
If a judge goes along with the plea agreement, Byzewski would serve three years of supervised probation, 60 days of electronic home monitoring and 50 hours of community service. She would be required to undergo a chemical-dependency evaluation and follow through with any recommendations.
If she satisfies the conditions of the plea agreement, she won't have to serve three years in prison, her guilty plea will be withdrawn and her case will be dismissed.
Investigators said Byzewski used the prescription pad of an NEHS psychiatrist, forged his signature and had the prescriptions filled at CVS Pharmacy in Grand Forks in August on two occasions.

In Polk County, Minn., she has pleaded guilty to a drug-related charge that stemmed from the same investigation. She's set to be sentenced there on Jan. 25.
Byzewski oversaw more than 20 state employees and led a program that serves people with mental illnesses. She resigned from her post in September.
